Adoption Application

Congratulations on taking the first step to becoming a canine parent to an amazing rescued companion!!! Please fill out and submit the following questionnaire to the best of your abilities.

Please note that due to the nature of our post-adoption support, WE ONLY ADOPT TO THOSE IN VERMONT OR IMMEDIATE SURROUNDING TERRITORIES (within 100 miles of Burlington, Vermont). 

 

We appreciate your understanding and compassion for our rescued canine kids!!! Please contact us if you have ANY questions regarding the application, our adoption process, or about the personality/lifestyle requirements of our adoptable companions <3

Our current adoption fee: $495

Included in adoption fee:

  • Up-to-date core vaccinations

  • Up-to-date heartworm and flea & tick preventation

  • De-worming

  • Spay/neuter

  • Microchip

  • Up-to-date on physical examination at local veterinary hospital

  • 2+ weeks (duration of VT foster-care) of quality nutrition including a 5 star rated dry food (Pronature Holistic) and added supplements including: fish oils, probiotics, multivitamins, fresh organic vegetables

  • 2+ weeks (duration of VT foster-care) of crate training

  • 2+ weeks (duration of VT foster-care) of basic behavior training (encouraging good manners, comfort being handled, and general doggy mindfulness)

  • 2+ weeks (duration of VT foster-care) of socialization with other canines

Please read our Adoption Contract prior to application! 

Your digital signature indicates that all of your information is accurate and complete to the best of your knowledge. Your digital signature also releases Rescue Me VT INC, and all of its representatives from any liability in reference to the adopted animal. You also agree that if you ever need to relinquish the animal during its life, you will contact Rescue Me VT INC for its surrender. Your signature also indicates that we have permission to contact your references. You understand and agree that this application is an expression of interest in adopting this animal, not an agreement or contract, and that applications are approved not on a first-come/first-served basis, but rather on criteria that provides for the best interest of the animal.
